For coverage to start January 1, 2014, Cover Oregon applications for health insurance and financial savings must be postmarked, faxed or submitted online by December 4, 2013.

Visit CoverOregon.com to get an application. Submit it online, or print it out and mail or fax it to Cover Oregon.

You can also call 1-855-CoverOR (1-855-268-3767) to request an application.

 Through a certified insurance agent or community partner, at no cost to you.
